To fill out required new patient paperwork, our patients will receive a secure text from their practice with a link to complete forms-no app or login is required.

For any new patients that prefer to fill out the paperwork on a desktop computer, they can complete the required forms by logging in at patient.klara.com using their phone number and date of birth (after they have sent them the eForm).
